COMPLETE RELEASE OF LIABILITY AGREEMENT: Having read this Waiver and knowing these facts, I, for myself, and anyone entitled to act on my behalf, waive and release the Organization for Autism Research, and its representatives and successors, officers, directors, members, agents, and employees from all present and future claims and liabilities of any kind, known or unknown, arising out of my participation of this event even though that liability may arise out of ordinary negligence or fault.
By clicking “Submit” above, I acknowledge that I understand its intent, and I for myself, my heirs, executors, administrators and representatives, do hereby agree and will absolve and hold harmless Organization for Autism Research, the sponsors, cooperating organizations and any other parties connected with this event in any way together with their respective successors and assigns, singly and collectively, from and against any blame and liability for any injury, harm, loss, inconvenience or any other damage of any kind whatsoever, which may arise out of the negligence or carelessness on the part of any person named in this waiver.
I am voluntarily entering this event and assume all risks associated with participation. I agree to abide by any decision of an event official relative to my ability to safely complete the event. I grant the Medical Director of the event and their designee access to my medical records and physicians, as well as other information relating to medical care that may be administered to me as a result of my participation in the event. I hereby consent to and permit emergency treatment in the event of injury or illness while participating in the Chip In Fore Autism - Topgolf National Harbor.
CANCELLATION POLICY: There are no deferrals, refunds, or cancellations for the Chip In Fore Autism - Topgolf National Harbor. Although no cancellations, refunds, or deferrals will be issued, if you are unable to participate in the event, you should reach out to the OAR staff as early as possible.
PHOTO RELEASE: I hereby further grant permission to OAR to use, or authorize others to use, any photographs, motion pictures, recordings, or any other record of my participation in the Chip In Fore Autism - Topgolf National Harbor for any legitimate purpose. By submitting this form, I hereby grant permission to OAR to use any photographs, motion pictures, recordings, or any other record of my participation in the event or their release, publication, exhibition, modification, or reproduction to be used for news, webcasts, promotional purposes, telecasts, advertising, including on websites and social media, or any other purpose, in perpetuity, by the Organization for Autism Research and its affiliates, partners, and representatives.
